Healthy Living with type 2 diabetes – face-to-face course booking form

Please complete this form to book a place on the Health Living with type 2 diabetes – face-to-face course. The face-to-face course includes two sessions, one week apart. Each session, led by a health professional, will last three-hours and will include other participants who are diagnosed with type 2 diabetes. You will need to attend both sessions.

The course is not suitable for:

  • People who do not have a diagnosis of type 2 diabetes
  • People under 18 years old
  • People who are pregnant
