Our service

Health Navigator is a telephone-based health coaching service delivered by registered nurses and health professionals, working as Health Coaches.

Proactive Health Coaching provides additional non-medical patient support to help patients understand their long-term health condition, plan their care and navigate an often-complex health care system.

We work with patients on a one-to-one basis and provide patients with a personalised coaching plan. We aim to reduce unplanned hospital care by empowering people to take more accountability, be more confident in managing their condition and use health care resources more efficiently.

Who do we support?

We work with patients with long term health conditions, such as COPD, Diabetes, Rheumatoid Arthritis and Heart related issues, from the age of 18. We select patients to be part of the service who have attended Torbay Hospital’s A&E recently.

How do we support patients?

  • Help co-ordinate and communicate with all health and social care services, working alongside other health professionals.
  • Help patients navigate the health care system.
  • Gain knowledge, skills, tools and confidence to improve patient’s health and quality of life.
  • Empower patients to reach self-identified health goals.
  • Help understand and manage condition, medications and treatments.
  • Address specific triggers that cause avoidable hospital attendance.
  • Identify the best NHS services to support your specific health needs.

How does it work?

Patients are selected to be part of the free service as they are identified as someone who could benefit from Proactive Health Coaching.

Patients then have their initial face-to-face assessment with their Health Coach to review the challenges they face, agree on some shared goals and make a plan for the future.

Over the next four to six months, the patient and Health Coach have regular contact over the phone to make progress in the patient becoming more confident in managing their condition and seeking the right care, at the right time. At the end of the coaching program, most patients feel confident to continue independently. By this time, the risk of seeking avoidable unplanned medical care has decreased significantly.
