Simple steps to keep you safe during your hospital stay.

Coming to hospital as an inpatient

  • Please confirm your appointment date.
  • Remember to bring the necessary belongings with you.
  • Do not bring valuables.

If you are scheduled for an operation, please remember not to eat and drink on the day. When you arrive you will be directed to out Surgical Admissions Unit or to your ward and settled in by a nurse. They will then help you complete your admission / consent form.

If you have any concerns regarding your treatment please ask the nurse / doctor treating you.

Hospital Passport for patients with learning disabilities

Please download the Hospital Passport and bring it with you to your appointment if you are going into hospital.

My Health Passport for autistic people

The Autism Partnership Board and Ambassadors have worked together to develop a health passport. Health passports are used to record:

  • information about yourself
  • your medical information
  • how you prefer to communicate with people

You can take your passport to medical appointments. This information can then be shared with health professionals.
