Health and wellbeing teams

The health and social care teams work with people in their own homes and care homes to be independent. They may recommend equipment, exercises, treatment and support with getting carers and help. The intermediate care teams work with people more regularly, over a few weeks, to support if they have more urgent needs and this can enable them to stay at home.

You can self-refer or with consent, refer a family member, friend or neighbour. Carers can have assessments in their own right as well.
