Accessing Health Connect Coaching as a peer: making a referral
A peer in the Health Connect Coaching programme is someone who may be feeling overwhelmed and challenged in the management of their long-term health condition/s, who would value and benefit from peer support to focus on what matters most to them, using a skilled coaching approach.
You will be carefully matched with one of our trained volunteer Health Connect Coaches, who will be living with the same or similar long-term health condition and/or symptoms, who will understand and be able to empathise with the challenges you are currently facing.
We match individuals with coaches based on information that is requested from you when you begin the application process. This information will include: health condition and/or symptoms, personal preferences (with regards to age, gender, culture etc.). We also consider health and social inequalities. We want you to have your say in the type of coach you feel would be best suited to you, which is why this information plays an essential role in our matching process. Our volunteer Health Connect Coaches are there to offer support using empowering coaching techniques, and will encourage you to take control of your health and wellbeing by supporting you to focus on ‘what matters to me?’
To refer into Health Connect Coaching to receive regular 1:1 coaching sessions with a matched trained volunteer, with lived experience of a long-term condition, please complete our online referral form or complete the below questions and someone from the contact the team will be in touch.
We accept self-referrals and those from a 3rd party e.g. health and social care provider or organisation.
Please note Health Connect Coaching is not a replacement for medical care, advice or treatment. It is a supportive ‘arm’ to talk with someone with shared experiences to help your ongoing management pathways and clinical interventions.
