Referrals

If you are a member of the public please make contact with the voluntary sector provider nearest to you and ask about the Wellbeing Programme. Please do not complete a referral form.

If you are a General Practitioner or health or care professional please click on the forms below and save to your computer before completing and sending to the relevant voluntary partner email address. To make a referral please:

• Gain the person’s consent for a referral
• Complete the referral using the relevant email friendly form below
• Send referral to the provider in the area where the person lives shown on the form

In my GP practice in Paignton we have Wellbeing Co-ordinators regularly attend our meetings each week, so they are really embedded and integrated into our team. In this way, all my GP colleagues are getting to know the Wellbeing Co-ordinators and are seeing first-hand just what a positive impact their interventions are having for many of our patients.
