Community Physiotherapy Service

If you wish to self-refer, or are a relative, carer or professional and wish to request a community Physiotherapy assessment, please call our customer service number on 01803 219700, who will direct you to the most appropriate team.

What we do

This service provides physiotherapy assessment, treatment and advice for people who are either unable to access the Hospital Physiotherapy Outpatient service, or for whom input in their home setting is felt to be more appropriate.

As part of the Intermediate Care Team, there is a service for people in crisis. For referrals for people whose requirements are less urgent, the priority is based on the individual’s needs.

Community Neurological Service

This service provides physiotherapy for people with neurological conditions such as Multiple Sclerosis, Stroke or a spinal cord injury.

There is access to specialist assessment and advice, and short spells of more intensive rehabilitation, with a goal of enabling people to manage their own condition. The Physiotherapists work alongside Occupational Therapists, Speech Therapists and rehabilitation support workers and will deliver care at home or in outpatient departments. The service includes an element of ‘early supported discharge’ for people with stroke, enabling them to return home faster with more intensive support.
