What happens after a referral

After your referral is received, our team will review your information and guide you through each step of your care. The list below explains what happens next.

  1. Your referral is received by our Health and Social Care Coordinators. This may come from your GP, another health professional, or through self-referral.
  2. Your referral is reviewed (triaged) by a clinician.
  3. You are added to our waiting list.
  4. We will contact you, or your next of kin or Power of Attorney, to arrange a telephone or Attend Anywhere appointment.
  5. A Physiotherapist will carry out your telephone or Attend Anywhere assessment. During this appointment, treatment options will be discussed.
  6. Depending on the outcome, you may have further virtual appointments or be offered a face-to-face appointment with the appropriate member of staff.
  7. Face-to-face appointments take place in your home or care home.
