FOI disclosure log

The Freedom of Information Act 2000 gives members of the public the right to request recorded information from public authorities. The purpose of the Act is to promote openness and transparency in how public services operate. As a member of the public, you have the right to know how organisations such as the NHS are organised and run, how services are delivered, how public money is spent, and how you can raise concerns or complaints if needed. We are committed to providing information wherever possible and to making it clear, accessible and easy to find. Before submitting a request, please review the information already available on our website and through our Publication Scheme and Disclosure Log. The Disclosure Log includes a search facility to help you find information that may already have been published in response to previous requests. If you cannot find the information you are looking for, you can submit a Freedom of Information request by emailing tsdft.foirequests@nhs.net or via our contact form.

Requests must be made in writing and include your real name and a correspondence address (email or postal address) so that we can process and respond to your request.

We encourage requests to be submitted by email as this is the quickest and most efficient way for us to process and respond to enquiries.
